Wayne County Birth Certificates

When you get an official, legal copy of your birth certificate… is it a card or is it a piece of paper? I have the yellow looking birth registration card that was issued by the register of deeds and I wanted to know if this is legit because I’m going to get my name changed in Guilford County and I don’t wanna walk in with a bogus document.
my birth registration card has all of the info that a birth certificate would. it was issued by the register of deeds… it has my name, my mother’s name, my father’s name, my dob, birthplace, sex, it has a seal on it and the signature of the issuer
